Schedule Your Initial Examination
Schedule your initial consultation with Dr. Bub using our convenient online scheduling below. Once your appointment is scheduled, you'll be sent a health history form to complete in advance of the appointment.
Program Options
At your consultation you can let us know if you are selecting the Pure Longevity Assessment, or the ongoing Pure Longevity Care program. If you aren't sure yet, that's fine! We can discuss your goals and which program fits your needs best at your visit.
Appointment Notes
If possible, please select a morning appointment and fast for 8 hours prior so that we are able to draw labwork during your visit. If you have any previous labwork results that you can bring with you we would love to see them!
